Please read the following information about receiving electronic notices from Affiliate Mortgage Services, Inc. If you consent to receiving electronic notices click below the "I accept" button.

I consent to receive any notices (includes disclosures, notices, statements, documents, agreements, emails, or other communication) regarding my mortgage application, pre-qualification or account delivered to me electronically. This includes any disclosures normally required to be made by law in connection with any service I may have or obtain in the future from Affiliate Mortgage Services, Inc. (AMS).

I also consent to AMS using my email address indicated below to communicate with me about my mortgage loan after the loan has closed.

I understand that when I agree to receive notices electronically, I may not be receiving these notices in paper form. I can withdraw my consent at no cost to me and begin receiving notices on paper or receive paper copies of notices I have received electronically, by contacting AMS using the contact information below. For purposes of receiving notices, I must provide a valid email address. If I change my email address, I will notify AMS of my new address.

I own or have access to a personal computer or similar device which provides me with the ability to receive electronic notices and on which I can retrieve email notices from AMS. I understand that in order to receive electronic notices from AMS, my computer hardware and software must meet the following minimum requirements: Windows 7, Internet Explorer 9, Acrobat 8.0. My computer must be capable of running this software.
